We have done our best to simplify the Georgia Fulton County Permitting Process. Below is a form to submit all necessary information. Please fill out in it’s entirety. You will need to attach your Government ID and BBP, First Aid, CPR certificate.

These responses will be used for us to complete your guest artist permit and hepatitis B vaccine declination.

Once your information is submitted a confirmation message will appear with a link to pay the $75 Guest Artist Fee. This fee goes up to $150 after 8/28/24.

Artists already permitted in Georgia can upload a copy of their current Artist Permit.

If you need a new BBP Certificate, we recommend this one, which is OSHA certified.
If you need a new FIRST AID Certificate, we recommend this one, which is OSHA certified.
If you need a new CPR Certificate, we recommend this one, which is OSHA certified.

$75 Temporary Permit Fee (link for payment will generate after this form is submitted. (artists with a Georgia Tattoo Permit do not need to pay this, attach a copy of your Georgia issued permit)
